Hospital discharge & transfer transport in Singapore — wheelchair-friendly, door to door

A booked private chauffeur for getting home from hospital, moving between hospitals, or crossing to a JB hospital — with a ramp vehicle if you stay in your wheelchair, or a driver who helps you into the car. Non-emergency, calm, and confirmed before you book.

Toyota Noah Welcab 1.8X Hybrid wheelchair-accessible MPV with rear ramp deployed and wheelchair

Leaving hospital should be the easy part of the day. But a discharge with a wheelchair, a drip stand, a bag of medication and a tired patient is rarely something an ordinary taxi handles well — and moving between hospitals, or across to Johor for ongoing care, is harder still. You need a vehicle that fits the chair, a driver who helps at both ends, and a time you can actually rely on.

We book non-emergency hospital transfers as a private chauffeur trip: hospital to home, ward to ward across two hospitals, or straight from a Singapore hospital to a JB facility in one vehicle. You tell us how the patient travels and when they’re discharged, we match the right vehicle, and we confirm everything on WhatsApp before anything is locked in.

TL;DR — Hospital transfers in 4 lines
Non-emergency, wheelchair-friendly transfers: hospital-to-home, hospital-to-hospital, or hospital-to-JB — door to door in one vehicle.
Stay seated in your own wheelchair on our ramp welcab, or have the driver help you into an MPV seat with the chair stowed.
Local transfers start from around S$65; cross-border trips to JB hospitals are quoted per booking.
This is transport, not an ambulance — for a medical emergency call 995.

When this is the right service

We arrange planned, non-emergency transfers for patients who are stable but need a suitable vehicle and a helping hand:

  • Hospital discharge to home — a calm, unrushed ride home with the wheelchair, luggage and equipment handled, timed around when the ward actually releases the patient.
  • Hospital to hospital — a planned transfer between facilities for a scan, a specialist, a step-down ward or a second opinion, where no ambulance is needed.
  • Home to hospital and back — for admissions, day surgery, dialysis and recurring appointments. For regular dialysis runs in particular, reliability is everything.
  • Singapore hospital to a JB hospital — door-to-door to KPJ, Columbia Asia and other Johor facilities, with no change of vehicle at the Causeway.
  • Step-down and nursing-home transfers — moving a patient to a care home or rehabilitation facility with dignity and the right vehicle.

If the passenger is elderly but walking, our elderly chauffeur service may suit; if the trip is to or from the airport, see Changi airport transfer; and for the wider picture on Johor clinics, see our JB medical tourism guide.

Hospital transfer, ambulance, or medical escort — which do you need?

This is the question families ask most, so let’s be clear. The right choice depends entirely on how stable the patient is.

ServiceBest forWhat you get
Hospital transfer (what we do)Stable patients who can travel sitting — in a wheelchair or a car seatA private chauffeur, a wheelchair-friendly vehicle, and help boarding. No medical care.
AmbulanceEmergencies, or patients who must lie down or be monitored en routeParamedics, stretcher, medical equipment. Call 995 for emergencies.
Medical escort transportPatients who need a trained nurse to travel with themA clinical escort plus transport — a specialised service we don’t provide.

In short: if the patient is stable and can travel seated, we’re a good fit and a fraction of an ambulance’s cost. If they need to lie flat, be monitored, or have a nurse along, that’s an ambulance or a medical-escort provider — not us.

How we move the patient: two options

There’s no single “accessible vehicle” that suits everyone, so we offer a choice and confirm the exact vehicle before you book.

Option A — Stay in the wheelchair (ramp welcab)

The patient stays seated in their own wheelchair, which rolls up a powered rear ramp and is secured inside. The vehicle we use for this is the Toyota Noah Welcab 1.8X Hybrid, a factory-built wheelchair-accessible MPV. This suits powered or non-folding wheelchairs and patients who can’t easily transfer to a car seat. The welcab is available on request, so the earlier you ask, the better we can match it. Full detail on our wheelchair-accessible transport page.

Option B — Assisted transfer into an MPV

The driver helps the patient out of the wheelchair into a regular MPV seat, then folds the chair and stows it. This works well for foldable chairs and patients who can transfer with a little support, and lets you ride in a comfortable MPV such as an Alphard or Vellfire. See our fleet for space and seating.

One honest limit: our ramp welcab is built for a standard wheelchair (around 100 kg). We don’t operate a heavy-duty hydraulic lift, stretcher or bariatric transport — if that’s what the patient needs, an ambulance or specialist provider is the right call, and we’ll happily say so.

Crossing to a JB hospital — one vehicle, door to door

This is where a private transfer really earns its keep. Many families move a patient to Johor for treatment because it’s closer and more affordable — and most Singapore wheelchair-transport apps simply won’t leave the island. We take the patient door-to-door in one vehicle, from the Singapore hospital or home straight to KPJ, Columbia Asia or another JB facility, with no transfer at the Causeway. Nobody wants to swap cars at the border with an unwell passenger.

Cross-border vehicles are MY-plated beyond JB and the whole journey is arranged as one booking. See our JB medical tourism guide for clinics and appointments, and our cross-border taxi to JB page for general runs.

What it costs

Local hospital-to-home and hospital-to-hospital wheelchair transfers within Singapore start from around S$65, with most local runs falling in the S$65–110 range depending on distance, the vehicle, and whether a ramp welcab is needed. Cross-border transfers to JB hospitals are a larger trip and are quoted per booking. Pickups between 11pm and 7am carry a flat S$30 night surcharge.

Because the right price depends on the patient, the route and the vehicle, we confirm a clear figure on WhatsApp before you commit — no surprises at the door. You can see our pricing page for general context.

How a discharge booking works

  • Message us on WhatsApp with the patient’s mobility (wheelchair type, can they transfer?), the route, and the expected discharge or appointment time.
  • We suggest the option (ramp welcab or assisted transfer), confirm the vehicle, and send a quote.
  • You approve, and we hold the slot around the discharge window — discharge times slip, so tell us a range and we’ll stay flexible.
  • On the day, the driver helps at both ends — boarding, securing or stowing the wheelchair, luggage and equipment — and takes the patient door to door.

Please give as much notice as you can. A little lead time lets us match a ramp vehicle properly rather than scramble — especially for cross-border trips, which need the right vehicle and a driver cleared for Malaysia.

What we are — and what we’re not

We want to be clear. We provide transport only — we are not an ambulance and we don’t provide medical care. Drivers help with boarding, steadying the patient, and luggage or equipment, but they are not trained medical escorts and can’t administer care during the trip.

If there is a medical emergency, please call 995 for an ambulance — not us. For planned discharges, transfers and appointments where the patient is stable, a private wheelchair-friendly chauffeur is the right, affordable fit.

FAQ

Is this an ambulance or medical transport?

No — this is non-emergency transport for stable patients who can travel seated. We provide a wheelchair-friendly private chauffeur, not paramedics, a stretcher, or medical care. For an emergency, or if the patient must lie down or be monitored, call 995 for an ambulance.

Can the patient stay seated in their own wheelchair?

Yes, in our ramp welcab — a Toyota Noah Welcab 1.8X Hybrid — the wheelchair rolls up a powered rear ramp and is secured inside, and the patient stays seated. It’s available on request, so tell us early. If the patient can transfer to a seat with help, an assisted transfer into an MPV is often simpler.

Do you do hospital-to-hospital transfers?

Yes — planned, non-emergency transfers between hospitals for scans, specialists, step-down wards or second opinions, where no ambulance is needed. We take the patient door to door and help at both ends. If the patient needs monitoring or to lie flat en route, that’s an ambulance instead.

Can you take a patient from a Singapore hospital to a JB hospital?

Yes — door-to-door in one vehicle, with no change of vehicle at the Causeway. We go straight from the Singapore hospital or home to KPJ, Columbia Asia or another JB facility. Cross-border vehicles are MY-plated beyond JB. See our JB medical tourism page for the wider picture.

How much does a hospital transfer cost?

Local wheelchair transfers within Singapore start from around S$65, with most local runs in the S$65–110 range depending on distance and the vehicle. Cross-border trips to JB hospitals are quoted per booking. Pickups between 11pm and 7am carry a flat S$30 night surcharge. We confirm a clear figure on WhatsApp before you book.

Discharge times always slip — can you stay flexible?

Yes. Discharge rarely happens to the minute, so tell us a likely window rather than a fixed time and we’ll work around it. The more notice you give, the easier it is to hold a suitable vehicle and adjust on the day.

Can a carer or family travel with the patient?

Yes. An MPV comfortably seats family plus a carer alongside the patient. Tell us the group size when you enquire and we’ll match a vehicle with the right number of seats. See our fleet for capacities.

Do you provide a wheelchair, hoist or stretcher?

No — please bring your own wheelchair. Our ramp welcab carries a standard wheelchair (around 100 kg); we don’t operate a heavy-duty hydraulic lift, hoist, stretcher or bariatric transport. If the patient needs any of those, an ambulance or specialist provider is the right fit and we’ll point you there.
