JB IVF and Fertility Guide for Singaporeans

IVF is one of the few JB-medical decisions where the cost answer isn’t always JB. SG citizens with full MediSave eligibility often find a SG public hospital cycle competitive or cheaper than the JB equivalent on a first attempt. JB IVF makes the strongest economic case for permanent residents and non-citizens without MediSave, for couples on a second, third or fourth cycle after the SGD $7,500 lifetime cap is exhausted, for donor cycles where SG access is restricted, and for couples who want anonymous treatment. We’ve written this guide to help you work out which side of that line you’re on, before booking either way.

The clinical side of the question is steady. JB and KL fertility centres run the same vitrification, ICSI, time-lapse incubators and PGT-A genetic screening you’d find at NUH or Thomson, with internationally trained reproductive endocrinologists and FDA / KKM regulated protocols. What matters more than the brochure is the clinic’s age-banded live-birth rate per started cycle — not pregnancy rates, not transfer rates. We come back to this further down.

The honest comparison — when JB IVF makes sense

Singapore’s MediSave framework subsidises IVF for SG citizens up to SGD $7,500 lifetime, with KK Hospital’s Centre for Assisted Reproduction running cycles at SGD $4,000–6,000 in subsidised pricing. NUH and SGH IVF run SGD $8,000–13,000 per cycle. Private SG centres (Thomson Fertility, Virtus Fertility) sit at SGD $12,000–20,000 per cycle. JB / MY private cycles land at SGD $5,100–8,900 all-in including stimulation medication.

Stack those numbers honestly. A SG citizen on a first cycle, drawing the full MediSave benefit, often comes out ahead at KK or a subsidised pathway. The arithmetic flips for several distinct groups:

• Permanent residents and non-citizens — MediSave eligibility doesn’t apply, so SG private is the comparison point. JB cycles save SGD $4,000–8,000 per attempt against SG private.
• Second, third and fourth cycles — the SGD $7,500 lifetime MediSave cap is exhausted after one or sometimes two cycles. From cycle two onward, cross-border savings compound to SGD $15,000–25,000+ over a multi-cycle treatment plan.
• Couples wanting anonymous treatment — privacy is a non-financial driver as significant as pricing for many couples. A discreet cross-border setup, with no SG-side referral trail, matters.
• Donor egg / donor sperm cycles — Singapore’s 2014 ART regulations require licensed centres and SG-registered donors only. MY permits known and anonymous sperm donation; egg donation requires per-cycle ethics committee approval. Many SG patients explicitly travel for donor work.
• PGT-A genetic screening cases — accessibility of pre-implantation genetic testing varies by SG centre and indication. Some couples find MY clinics easier to access for PGT-A on non-medical indications.

If you’re a SG citizen under 40 doing a first cycle with no donor or PGT-A consideration, the honest answer is to get a quote from KK Hospital first. The JB conversation makes more sense from cycle two or for the categories above.

Top JB and nearby fertility centres SG couples use

Sunway Fertility Centre (Sunway Medical Velocity, Tebrau)

Opened 2022 within Sunway Medical Velocity — the same JCI-accredited hospital we cover on the JB health screening guide. Modern lab and IVF suite, ICSI, IMSI, embryo vitrification and PGT-A genetic screening on menu. Clinic-published live-birth rate at 35–37 years runs roughly 38–45% per fresh cycle. Indicative cycle cost: MYR 18,000–28,000 (SGD $5,700–8,900) all-in for a standard ICSI cycle including stimulation medication. Penang IVF Centre (Loh Guan Lye Specialists, Penang)

Higher-volume centre in Penang’s longer-established fertility cluster. Live-birth rate at 35–37 years runs roughly 42–48% per fresh cycle — the volume effect is real for IVF outcomes. About one hour by flight from Singapore; some couples consolidate visits by clustering monitoring scans into longer Penang stays rather than daily SG↔Penang. Cycle cost: MYR 16,000–24,000 (SGD $5,100–7,600).

Genesis Fertility Centre KL (Cheras, Kuala Lumpur)

Specialist-led, high reputation in the KL fertility scene. About four hours’ drive from Singapore — most SG couples doing Genesis cycles base in KL during the stimulation week. Cycle cost: MYR 18,000–26,000 (SGD $5,700–8,300).

Indicative pricing — JB / MY vs Singapore

SGD-equivalent ranges as of early 2026. Cycle pricing varies by stimulation protocol, medication dose, lab add-ons (ICSI vs conventional IVF, PGT-A, time-lapse imaging) and frozen-embryo storage. Always get a written package quote.

The numbers above are headline cycle quotes. Add SGD $1,000–2,500 for additional stimulation medication if your protocol calls for higher dosing, plus SGD $400–800/year for embryo storage if you’re freezing surplus embryos. Live-birth rates matter more than pregnancy rates

This is the single most important slide in the IVF brochure that often goes unread. Clinics quote three different success metrics and they’re not interchangeable:

• Pregnancy rate per transfer — the highest number, the easiest to publish, the least useful. A positive pregnancy test isn’t a baby.
• Clinical pregnancy rate per transfer — better, but still excludes miscarriage and chemical pregnancies.
• Live-birth rate per started cycle — the only number that maps to “we walked out of the hospital with a baby”. This is the metric to ask for.

When you interview a fertility clinic, ask specifically: “What is your live-birth rate per started cycle, banded by maternal age, for the most recent reporting year?” The age bands matter — IVF success rates fall sharply after 35 and again after 40. A clinic-wide average dragged up by a young patient panel tells you nothing about your own odds at 38.

Reputable clinics publish age-banded live-birth statistics in their annual reports. If a clinic only quotes “pregnancy rate” or refuses to break the data out by age, that’s a flag. The named MY centres above (Sunway, Penang IVF Centre, Genesis) all publish age-banded data; KK, NUH and the major SG private centres do likewise. Compare like-for-like before deciding.

Multi-visit cycle logistics — what 6 to 8 weeks actually looks like

An IVF cycle is not a one-day visit. A typical fresh stimulation cycle runs across about six to eight weeks and 8 to 10 SG↔JB trips:

• Visit 1 — Initial consult, AMH and FSH bloodwork, transvaginal ultrasound. About 2 hours at the clinic.
• Visits 2 to 6 — Stimulation phase monitoring. Every 2–4 days for follicle-tracking scans and bloodwork. Five visits or so across about 10–14 days.
• Visit 7 — Trigger injection and egg retrieval. Day surgery under IV sedation; about half a day at the hospital.
• Visit 8 (3–5 days later) — Embryo transfer. Out-patient, about 30 minutes.
• Visit 9 (10–14 days later) — Pregnancy beta-hCG blood test. Quick blood draw plus result review.
• Optional visits 10+ — Follow-up scans at 6 and 8 weeks gestation if pregnancy confirmed.

The stimulation week is the punishing part — daily or every-other-day cross-border for monitoring scans isn’t comfortable. Many couples stay at a Tebrau or Iskandar Puteri hotel for 5 to 7 nights during stimulation, then return to SG between trigger and transfer. This shape works well with a chauffeur on retainer for the morning monitoring runs and an SG return at the end of the stimulation phase.

Donor cycles, PGT-A and what’s regulated differently

Singapore’s 2014 ART regulations restrict donor egg and sperm to SG-registered donors at licensed SG centres. For couples who can’t find a suitable SG-registered donor — donor scarcity is a real constraint at the SG side — JB and broader MY centres are often the practical pathway. Malaysia permits both known and anonymous sperm donation under KKM regulation; egg donation is permitted with per-cycle ethics committee approval.

PGT-A (pre-implantation genetic testing for aneuploidy) is available at both SG and MY centres but with different indication thresholds and costs. MY centres are typically more accessible for non-medical PGT-A and run roughly 30–50% under SG private pricing per embryo screened.

Planning the trip from Singapore

Most monitoring days fit a half-day window: 6:30–7:00 AM SG pickup for a 7:30–8:00 AM clinic arrival, scan and bloodwork by 9:30, late-morning return to SG. Egg retrieval day is a full-day hospital commit — early-morning sedated procedure, post-op observation, mid-afternoon discharge. Embryo transfer day is shorter but you’ll want comfortable transport and a relaxed afternoon after the procedure.

Vehicle-wise, a Standard MPV (Toyota Voxy class) is the comfortable default for the couple — quiet cabin for early-morning monitoring days and space to recline post-retrieval. • Do the SG-MediSave maths first. If you’re a SG citizen under 40 on a first cycle, KK Hospital’s subsidised pathway is often cheaper than JB. Get both quotes before deciding.
• Ask for live-birth rate per started cycle, banded by age. Pregnancy rates and transfer rates flatter the brochure but don’t predict your outcome.
• Bring SG fertility records. HSG results, semen analysis, hormone panels — duplicate testing wastes a cycle’s worth of time and budget.
• Plan the stimulation week realistically. Daily cross-border for 10–14 days is exhausting. Hotel-stay during stimulation is often the kinder shape.
• Confirm SG insurance reimbursement in advance. Many SG plans exclude IVF entirely or cap coverage tightly — verify with the insurer before committing.
• For couples over 40, consult a SG specialist alongside the JB quote. Success rates fall sharply after 40 and the right call may involve different protocols, donor egg pathways or stopping rules.
• Frozen embryo storage matters. If you’re freezing surplus embryos, confirm the clinic’s storage terms, annual fees and what happens on relocation back to SG.
